Resolution approving acceptance of funds from the Department of Health Care Services under the Capacity and Infrastructure, Transition, Expansion and Development Program, which is part of the California Providing Access and Transforming Health Initiative Round 3 funding, for the purpose of supporting the Enhanced Care Management team in the maximum amount of $355,663.99 for the term of September 1, 2024, through August 31, 2026, authorizing the Director of Behavioral Health to sign this agreement and all related documents as required by the state, and directing the Auditor-Controller to amend the Behavioral Health budget for fiscal year 2024/25.
