Legislative Title:

Resolution approving an agreement between the County of Nevada and Partnership Health Plan of California for the provision of Enhanced Care Management services, which will automatically renew at 1-year intervals until either party terminates the agreement, authorizing the Chair of the Board of Supervisors to execute the agreement, and authorizing the Director of Behavioral Health to execute the Certifications and all associated documents and amendments.
